Key considerations

The first questions are about urgency, governance and preservation — before collection expands or interviews begin.

  1. What was reported, through which channel, and is there an immediate safety, retaliation, evidence-loss or reporting risk?
  2. Does anyone who normally receives or investigates the allegation have a conflict or reporting-line dependency?
  3. Which employment, privacy, criminal, anti-bribery or financial-control owner must join the intake without widening disclosure?

Direct answer

What this page decides

Owner
02

This page owns the reporting channel after an allegation arrives: classification, anonymity, retaliation risk, independence of the investigator, and closure hygiene. It does not decide whether the facts are bribery, how to image a phone, or how to dismiss someone. Those stay with Commercial Bribery, Internal Investigations and Employment. Malicious reports still need a defensible intake — do not skip preservation because the report ‘looks political’. Criminal referral is a later gate, not the first email.

Common failure points

Where files usually break

  1. Sending the report to the named manager ‘to comment’.

  2. Promising anonymity the channel cannot keep.

  3. Closing as ‘HR issue’ without a preservation hold when money moved.

  4. Using the investigation to punish the reporter.

  5. Ignoring a malicious-looking report instead of logging it.

  6. Mixing findings, gossip and dismissal in one email.

  7. Updating the whole department on ‘who reported’.

Moments where sequence matters

01

Anonymous invoice fraud tip

Preserve finance and messenger data before asking the AP team who they think wrote the email.

02

Named junior reports a VP

Independence and retaliation are the first facts. Do not assign the VP’s HRBP as investigator.

03

Repeat complaint already ‘closed’

Re-open intake. Prior closure is not a finding that the new facts are false.

FAQ

Narrow follow-ups owned by this page

Must we investigate every anonymous report?

You must intake and triage. Scope follows risk, not the reporter’s tone. Do not invent a statutory investigation duty that is not in the file.

Can we tell the accused who reported them?

Default is no. Need-to-know only, with a retaliation analysis.

What if the report is defamatory?

Still preserve. Malicious reports are an employment/legal track after facts, not a reason to skip the log.

When does employment take over?

After there is a fact record sufficient for a disciplinary process. This page does not calculate severance.

Decision map

Keep the question bounded.

  1. Acknowledge and protectCreate a controlled intake record, identify contact and confidentiality preferences, assess urgent harm and document practical measures against retaliation.
  2. Check independenceRoute around implicated managers, conflicted functions and reporting lines. Define who may see the allegation, approve scope and receive findings.
  3. Preserve proportionatelyProtect the records most at risk without treating the report as proven or launching indiscriminate monitoring. Coordinate employee-data access with the privacy and employment owners.
  4. Separate findings from disciplineDocument facts and credibility first. Route warnings, suspension, termination, recovery, reporting and control remediation through their distinct legal and governance tracks.
